congresocrónicos-kronikgune-proyectos-investigacionThis year the motto was “Making Progress with Results”, with the aim of applying the theory to healthcare practice, making this year’s congress a meeting point to work on new challenges and define the steps required to address the challenges posed by chronic illnesses in healthcare systems.

New experiences and projects related to the main chronic strategic lines were presented this year and progress was made in the start-up of care models aimed at improving healthcare focused on chronic patients. In this regard, the European Projects where Kronikgune works together with Osakidetza had a prominent presence in oral communication sessions organised during the event.

Thus, the new healthcare pathway implemented and Kronik ON empowering programme developed within the Carewell programme and implemented in different Osakidetza organisations were presented to the attendees by Marisa Merino, OSI Tolosaldea Manager and main European project researcher. Furthermore, it is worth mentioning two presentations made on the United4Health European project, exhibited and presented by Kronikgune: one from the viewpoint of Osakidetza professionals and the other from healthcare service patients. This study has facilitated the assignment of innovative telemedicine solutions within Integrated Healthcare for Heart Failure patients within the region.

Likewise, MasterMind and ASSEHS European projects were also presented in a brief communication session. The first speech focused on the implementation of Cognitive-Behavioural Therapy (CBT) for patients suffering from depression while the second presented the hurdles and facilitators identified in the implementation of Population Risk Stratification in the Basque Country.

Finally, the European study Pre-Start was presented in poster format on the congress touchscreens.

The 8th National Congress of Chronic Patient Healthcare is an annual meeting point for public and private healthcare professionals in PrimaryHospitalMental Health and Social Healthcare, where attendees had an opportunity to obtain first-hand knowledge of the European research projects where Kronikgune and Osakidetza are involved.
