Kronikgune and Osakidetza have made an information video about the Carewell project to tell the general public about the work carried out in the Basque Country.
The result of the Carewell project in the EC’s annual review was excellent. In fact, one of the things that the EC recommended was to let the general public know about Carewell. With this in mind, Kronikgune and Osakidetza (the Basque Health System) have produced an information video to present the aim of the project, the new integrated healthcare pathway for patients with multiple comorbidities, the experience of healthcare managers, clinical staff and patients, and the results of the Carewell assessment in the Basque Country.
This video also aims to show the hard work of all the people involved in the project and who helped to develop and implement it. A multidisciplinary team has been working on the Carewell project, made up of researchers and project managers, managers of integrated healthcare organisations, medical and nursing professionals, social workers, etc. They have all joined forces to work for the most important part of the Carewell project – the patients and their carers. Thanks to all the people involved a new integrated care model for frail elderly patients has been implemented and assessed.
Carewell is currently in its final stage. After defining and implementing a new integrated care model, the results are being assessed in all the regions – Apulia (Italy), Veneto (Italy), Zagreb (Croatia), Lower Silesia (Poland), Powys, Wales (UK), and the Basque Country. The results of this assessment will provide the scientific evidence that is required to assess its cost-effectiveness, which is key when deploying the healthcare pathway.
