With the slogan “Primary Care: Showing the Way”, the Spanish Society of Primary Care Managers (SEDAP) held its XVII Congress, the objective of which was to share and provide a global vision of the profession and to debate the leadership in the public health model that primary care must take on, in Palma de Mallorca.

premio-proyecto-carewell-congreso-sedapThe meeting brought together 300 professionals from the sector, and representatives of both Kronikgune and Osakidetza oral presentations concerning two of the European projects in which they are involved, namely, Carewell and ASSEHS; there was also a poster concerning the Pre-Start project. The communication about the European Carewell project, entitled “Implementation of integrated care for multiply co-morbid patients in the Basque Country“, presented by Dr. María Luisa Merino, Managing Director of the OSI Tolosaldea, was awarded the best oral communication prize at the conference. A new prize awarded to Carewell, the objective of which is to define, evaluate, implement and validate new and cost-effective integrated healthcare models for complex chronic patients via different pathways, such as the improved coordination of healthcare professionals, promotion of home-based patient monitoring and promotion of the ability of patients and caregivers to self-manage the disease.

These interventions aim to have a positive impact on care quality and efficiency and the satisfaction of both professionals and patients alike. An educational programme, known as the Kronik ON Programme©, which involves sessions to promote disease awareness and control, has been developed in the framework of the project. Educational materials that will be issued to patients/caregivers and which will also be made available in the Osakidetza section of the Osasun Eskola. web site has also been prepared. In addition, the patients concerned can access all information via their Health Dossier.

This project seeks to promote innovation in patient care using a new empowerment method involving the use of both in-person and distance channels.
