World Health Day is a health awareness day celebrated every year on 7th April. This year’s theme focuses on the importance of healthcare, with the slogan “Health for all”. The European Commission, together with the countries of the European Union (EU), is working to improve health systems in the EU and to ensure equal access to modern and efficient healthcare for all citizens. To this end, the European Executive Agency for Health and Digitisation, HaDEA, has funded projects and initiatives that address health inequalities by developing effective strategies and innovative technologies to facilitate access to healthcare. HaDEA wanted to refer to some projects and initiatives that promote the transformation of health and care systems aimed at people, including the JADECARE project, which is coordinated by the Biosistemak Institute (formerly Kronikgune).

Funded under the Third Health Programme, the Joint Action on implementation of digitally enabled integrated person-centred care (JADECARE) aims to support health authorities in the transition towards more digital, integrated, and personalised care. To this end, the project is transferring and adopting four Best Practices (from the Basque Country, Catalonia, Denmark, and Germany) selected by the European Commission to different contexts and health systems in the EU. This project aims to help reduce health inequalities in the EU and encourage the implementation and use of digital and innovative solutions to deliver person-centred healthcare.
