The aim of this programme is to generate knowledge that will allow us to improve the health of the population as a whole by studying the factors that determine chronicity, the use of stratification models and health equity. This includes a study of the determining factors involved in chronicity, stratification models and an analysis of the complexity and comorbidity levels of the population.
Results: The identification of “target” patient groups for specific interventions, the development of models and segmentation of the population in order to plan the resources required to meet the different needs in an adaptive and creative manner.
